Community Paramedicine

The Cochrane District Services Board’s (CDSB) Community Paramedicine (CP) Program is a part of Paramedic Service focused on prevention instead of response. 

CP paramedics use their medical skills and experience to help identify and monitor medical issues for parents in their home or community setting, before they require urgent or emergency care. In partnership with other health care professionals and social services, our CP paramedics help connect patients with the best-fitting supports. This program aims at keeping people well in their home or community setting, helping reduce pressure on our health system. 

CDSB’s CP Program supports:

  • Improving the wellness and quality of life for our patients
  • Reducing the risk of medical issues getting worse, requiring 911 response
  • Reducing emergency department visits and hospital admissions
  • Supporting our community members and empowering them to stay well in their home or community environment
  • Helping coordinate and navigate supports with our patients, their caregivers and loved ones

Home Visits

During a home visit, a CP paramedic may:

  • Ask about the patient’s health, mental health or social (support) conditions
  • Check the patient’s blood pressure 
  • Evaluate a patient’s mobility (ability to walk/move around)
  • Review medications
  • Identify any hazards in the patient’s home or physical environment (tripping hazards, access to shower/washroom spaces, food storage or preparation areas)

The visits are to make sure that patients are safe and healthy in their home or physical environment. 

Like with any health service, the information collected and shared to the Community Paramedicine paramedic is private and confidential. Client consent is required before personal information is shared, or a referral is made (other than CDSB Paramedic Service). Referrals

The CDSB Community Paramedicine Program is a referral-only service. Your primary care giver (family doctor or nurse practitioner) can complete a referral to the Community Paramedicine Program.

Wellness Clinics 

Through the CP Program, Wellness Clinics have been held in numerous communities throughout the Cochrane District. Wellness Clinics provide patient assessments, including taking vitals, which are recorded in the patient’s pocketbook, supplied by the CP Program. 

Community Paramedicine Remote Patient Monitoring Program

The CP Remote Patient Monitoring (CPRM) Program provides home monitoring for patients with Chronic Obstructive Pulmonary Disease (COPD) or Chronic Heart Failure (CHF). 

CPRM patients are taught how to take their vitals and that information is shared by email to the CP Program paramedics. 

Our experienced paramedics enjoy sharing their knowledge and helping build healthy, safe communities across the Cochrane District. Based on this information, paramedics will follow up with patients to discuss their health and make a home visit if required. 

With patient permission, CP paramedics can also refer patients to other services like home care supports, diabetic services, Meals on Wheels, etc.
