SUMMARY
The CDPS CP Clerk (Care Navigator) will support the CDSB vision by working with the CDPS team, under the direction of the Commander of Community Paramedicine, to ensure that the office operations run smoothly and are successful in supporting the administration of the service in accordance with CDSB policies and procedures and CDSB Vision & Values.

KEY RESPONSIBILITIES OF POSITION
- Receive and evaluate referrals from Primary Care Providers, Home and Community Care Support Services, community caregivers, and self-referrals to determine the best pathway for response.
- Triage and assess client eligibility for Community Paramedic Services based on organizational policies and relevant legislation.
- Assign, distribute and maintain Community Paramedic Practitioner caseloads and appointment schedules based on organizational policies and best practice.
- Contact clients to schedule appointments and provide appointment reminders.
- Complete and forward HCCSS and other community service referral forms.
- Monitor referral outcomes within established timelines to ensure the appropriate community support services have been offered by direct communication with the relevant service or agency.
- Forward Community Paramedic Practitioner progress notes, diagnostic results or other requested documentation via fax or electronic medical record to partner service or agency. Update clinical information by scanning all relevant material into client charts, informing team members as necessary, creating reminder calls lists, responding to provider requests, and by organizing and conducting scan audits.
- Manage telephone calls and faxes, relay messages, and answer general Program enquiries.
- Conduct regular maintenance of electronic files including updating information such as phone numbers, addresses, and resource names.
- Perform queries of client demographics and Program progress utilizing Microsoft Access or SQL.
- Generate statistics from electronic medical records for program development and evaluation, workload measurement and quality improvement.
- Prepare reports for the Ministry of Health & Long-Term Care, Ontario Health, and the CDSB as required.

JOB REQUIREMENTS
- Demonstrate proficiency in medical terminology.
- Demonstrate effectiveness working with confidential and sensitive information.
- Demonstrate knowledge of patient care computer systems/software programs (i.e., Health Partner Gateway, Clinical Connect, Radius, Secure File Transfer Service)
- Demonstrate intermediate proficiency in Microsoft Office Suite of products and ability to learn new software programs.
- Demonstrate excellent verbal and written communication skills, with skill in telephone interviewing and report writing.
- Demonstrate professional work attitude, behavior, and communication in interaction with internal and external stakeholders and dealing with clients in a sensitive and tactful manner.
- Demonstrate effectiveness working with minimal supervision with proven self-initiative, reliability, and trustworthiness.
EDUCATION & EXPERIENCE
- Medical Office Administration Diploma or Certificate, or equivalent education and/or experience.
- Two years of relevant experience or a combination of education/work experience satisfactory to the employer.
