Cochrane District System of Care

The Cochrane District System of Care refers to our regional coordinated access system. In practical terms, the CD System of Care is a mechanism that allows various agencies to work together to best support people who are trying to secure and maintain housing.  The CD System of Care will also provide a transparent, fair and streamlined approach to access to housing and services for people experiencing homelessness in our community.  The CD System of Care is committed to eliminating chronic homelessness and building a District where everyone has a safe and affordable home in a neighbourhood they choose to live in.

Reaching Home, Canada’s Homelessness Strategy, is a community-based program which provides direction and support the coordinated access system.  The goal of Reaching Home is to provide to the most vulnerable Canadians the supports required to maintain safe, stable and affordable housing and to reduce chronic homelessness for each community.  For additional information, please visit Infrastructure Canada – About Reaching Home: Canada’s Homelessness Strategy.

The Community Homelessness Report (CHR) is part of Canada’s Homelessness Strategy to help communities transition to a outcomes-based approach.  The CHR provides the community a yearly picture of the state of homelessness while bringing to light the results of the collaborative efforts across the community.
